The spleen gets enlarged due to the back pressure in the portal vein… Remember that the portal vein is formed by the Splenic vein and the superior mesenteric veins. So any obstruction in the portal vein blood flow will cause obstruction in the blood flow in the Splenic vein as well. This causes congestion and enlargement of the Spleen.
Same situation happens in Carcinoma or Cirrhosis of the liver. The Carcinoma obstructs the portal vein blood flow, and causes an enlarged spleen. An enlarged spleen is the cardinal clinical finding in a person with Portal Hypertension

Back-pressure will cause spleen to increase in size because the blood gets pooled there.
Just imagine the portal system as plumbing or a network of pipes through which water flows, and the spleen would be at the end of the pipe, like a shrunk sponge. When there is a block in the pipe, the back pressure will cause the sponge to soak up more water, and in the process expand the sponge.. wouldn’t it? Similar analogy…
